LINDA SMITH

BSN, RN, CMC, CDP

FOUNDER | CEO | REGISTERED NURSE, ADVANCED CERTIFIED LIFE CARE MANAGER, CERTIFIED DEMENTIA PRACTITIONER

Linda has her bachelors in nursing for many decades, is a certified life care manager for over a decade, certified dementia practitioner and nurse life care planner.

 

Her experience with caring for adults and seniors spans more than three decades. She uses the nursing process of assessment, critical thinking, and nursing diagnoses along with specific skill sets inherent to the determination of treatment needs and respective costs, across the continuum of care.

 

She provides coordination of patient care, construction of care plans and consultative services to families and professionals to determine treatment needs and respective costs across the continuum of care. Her tireless capacity to develop creative solutions results in positive outcomes for her clients. She is knowledgeable on a wide range of medical, psychiatric, disability, geriatric conditions and community resources.   Linda strategizes with medical providers, social service agencies, case managers, estate attorneys, financial planners and other professionals in order to maximize services.  

 

After receiving her bachelor of science in nursing from Simmons College in 1985, Linda gained a broad range of experience at Massachusetts General Hospital as a post operative surgical nurse, insurance quality and utilization review nurse at Blue Cross Insurance Co., then over 10 years nursing clients with complex medical and surgical conditions back to health along with assessment, evaluation, teaching, and medication management for Medicare certified home health agencies.

 

She and her team are continually learning and staying abreast of changes in the healthcare & homecare industry. They offer stimulating panel educational events for professional groups and offer fresh perspectives regarding navigating the health care system, such as how to be an effective family advocate, how to plan for successful aging and more.

 

Born and raised in Weston, MA she and her husband/business partner have raised two adult children. When she isn't writing care plans, you can find her indulging in her love of country dancing, yoga, biking and color analysis.

DEBORAH SMULIANL Social Worker

Deborah Smulian
LCSW., BC- DMT., LMFT., CGP

Social Worker

Commitment to Family Caregivers: A core component of Deborah's practice is supporting the family system. She actively teaches family caregivers practical strategies to reduce stress, prioritize their own well-being, and effectively share the often-heavy responsibility of caregiving. Her goal is to create sustainable, positive care environments for both the client and their loved ones.

Raised in En​gland, Deborah moved to Israel as a teen to become a professional dancer and then settled in Boston and became a licensed social worker. She worked as a hospital case manager doing care coordination, leading support groups and discharge planning. She counsels people with addiction, anxiety, depression and leads support groups for cancer survivors.  Her wit, charm and introspection helps h​er clients find meaning in their lives.

Cherise DeFeo BSN, RN

Care Manager

Cherise is a highly versatile Registered Nurse with nearly two decades of experience spanning acute hospital care, private practice management, and specialized clinical nursing. Since earning her Bachelor of Science in Nursing from Salem State College in 2006, she has built a reputation for clinical excellence and compassionate patient advocacy.

​

Her diverse professional background includes serving as a Charge Nurse on the Medical-Surgical unit at Hallmark Health Hospital, where she managed high-acuity bedside care and led nursing teams in a fast-paced environment. Cherise has also held leadership and administrative roles as a Nurse Manager for both Allergy and Plastic Surgery practices, balancing complex office operations with direct clinical interventions.

Laura Grome BBA, RN

Care Manager

Laura Grome is a trusted expert and Registered Nurse dedicated to guiding families through the complexities of elder care with clinical authority and unwavering support. With an extensive background as a Hospital and Home Care Liaison Case Manager, Laura excels at navigating hospital admissions, optimizing facility stays, and managing seamless transitions between care settings. Her expertise also includes conducting cognitive and functional assessments to help families maximize long-term care insurance benefits and secure essential services.

Beyond clinical coordination, Laura is a compassionate partner who prioritizes patient autonomy and creative problem-solving. She takes a holistic approach to care, offering meticulous attention to detail through services like home organization to create safe, calm environments for those with memory illness. By combining practical healthcare expertise with deep empathy, Laura’s mission is to alleviate the burden on family caregivers while ensuring older adults enjoy the highest possible quality of life.

Director 

Bob has a life long career in sales and management, develops new business relationships and establishes processes for operations. He has witnessed his wife’s tireless devotion to her clients for decades. "We pride ourselves on our ability to design individualized solutions which results in positive outcomes.”

 

Born and raised in Chestnut Hill, Newton, a graduate of Penn State, previous corporate wholesale business owner. His numerous interests include cycling, cooking and camping in the great outdoors.

Sophie Kiiza Doherty BSN, RN

Care Manager

Sophie is an exceptionally positive and well-rounded Care Manager, distinguished by her unique blend of clinical, administrative, and financial expertise. Her professional background in nursing, accounting, and home care administration—combined with her compassionate nature and optimistic outlook—allows her to address complex client needs holistically.

Client-Centered Approach: Sophie provides comprehensive support by skillfully evaluating client issues, offering effective counseling, and providing necessary education. She excels at connecting clients and families with critical elder care resources tailored to their specific situations.
